Innovaccer, a healthcare technology and AI company, has announced its acquisition of Story Health, a digital specialty care platform. The move, revealed on Tuesday, aims to enhance Innovaccer's Healthcare Intelligence Cloud offering and expand its reach in the specialty care market. Financial terms of the deal were not disclosed.

Strengthening Continuous Care Through AI and Remote Monitoring

Story Health, founded in late 2020, has made significant strides in combining virtual coaching, biometric monitoring, and care team alerts to support patients with chronic conditions between clinical visits. The platform has demonstrated impressive results in partnership with several health systems and insurers, including Intermountain Health, Saint Luke's, ChristianaCare, and Medicare Advantage insurer Zing Health.

Abhinav Shashank, co-founder and CEO of Innovaccer, emphasized the importance of this acquisition, stating, "Healthcare doesn't change through dashboards alone. It changes when data and AI power completely new clinical models. Story Health has proven that in specialty care, and we're excited to bring this technology and clinical expertise to our health system customers nationwide."

Proven Outcomes and Cost Savings

Story Health's approach has yielded significant improvements in patient care and cost reduction. A pilot program involving 35 patients at Intermountain Health resulted in approximately $210,000 in cost savings, below-average hospital readmission rates, and 10 hours per month of freed-up clinic appointments per 20 enrolled patients.

More broadly, provider partners utilizing the Story Health platform have reported a 6.9% 30-day heart failure all-cause readmission rate, compared to the national average of 18.1%. Additionally, hospital rate reductions of more than 60% have been observed, along with improvements in patient engagement and medication adherence.

Integration and Future Plans

Innovaccer plans to integrate Story Health's platform into providers' electronic health records and existing workflows. The company also intends to incorporate agentic AI capabilities alongside Story Health's live coaches, aiming to scale the care model to more providers and plans across the country.

Tom Stanis, co-founder and CEO of Story Health, expressed enthusiasm about the merger, saying, "Joining forces allows us to scale nationally and turn continuous, AI-assisted specialty care into the new normal."

Existing Story Health customers can expect uninterrupted service, and the company still plans to expand into other clinical specialties such as diabetes and chronic obstructive pulmonary disease.

Dr. Kirk Garratt, medical director of the Center for Heart and Vascular Health and chair of cardiology at ChristianaCare, a Story Health partner, commented on the potential impact of the integration: "The integration with Innovaccer will allow us to scale this model further, empowering our teams to provide continuous, personalized care without adding to clinician burden. This is a critical step forward in making advanced, AI-enabled specialty care accessible to more patients in our community."
